In consideration of my engagement as a subject and for other good and valuable consideration herein acknowledged as received, I hereby grant the following rights and permissions to Chameleon Studio Photography, their legal representatives, and assigns, those for whom Photographer is acting, and those acting with their authority and permission. They have the irrevocable, perpetual and unrestricted right and permission to take, use, re-use, publish, and republish photographic portraits, pictures and/or video of me or my athlete without restriction as to changes or alterations, in conjunction with my own or a fictitious name, or reproductions thereof in color or otherwise, made through any medium at his/her studios or elsewhere, and in any and all media now or hereafter known, specifically including but not limited to print media and distribution for illustration, promotion, editorial, or advertising. I understand complete creative control is left to Chameleon Studio Photography. I hereby release, discharge, and agree to hold harmless Chameleon Studio Photography, his/her heirs, legal representatives, and assigns, and all persons acting under his/her permission or authority, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of such photographs or in any subsequent processing of them, as well as any publication of them, including without limitation any claims for libel or violation of any right of publicity or privacy.

Photo Session Agreement *

High School Senior Banner & Photo Session Agreement By signing below, I acknowledge and agree to the following terms and conditions regarding my athlete’s participation in the banner photo session with Chameleon Studio Photography: 1. Creative Control & Image Use I understand and consent to the digital compositing and editing of portraits for banner creation. I acknowledge that Chameleon Studio Photography maintains full creative control over all aspects of the photo session and final images. All images will be used and shared in a respectful and tasteful manner. 2. Session Participation & Responsibility I understand that the quality and variety of images depend on the athlete’s willingness to participate. It is the responsibility of the parent/guardian and athlete to ensure full participation in all assigned poses during the session. Refunds will not be issued if the athlete fails to fully participate. 3. Images & Deliverables Each athlete will receive three (3) images. Additional images may be included with added payment. Photo sessions must take place during the scheduled banner session date and time. No reshoots or retake dates will be offered. Images will be delivered via an online digital gallery. Images intended for the banner will be delivered on a solid background. 4. Design & Product Policies Banners will be delivered approximately 14 business days after the session, pending timely approval of the final design. BigHeads and yard signs must be ordered no later than the day of the session. Additional banner design requests and/or late banner orders are subject to extra design fees and shipping costs. 5. Digital Files & Refund Policy RAW or unedited digital images will not be provided under any circumstances. All payments for photo sessions, banners, yard signs, BigHeads, or other products are non-refundable and non-transferable. 6. Authorization By signing or checking the agreement box, I affirm that I am of legal age and/or the parent or legal guardian of the subject. I have read, understood, and agree to the terms outlined above.
